Student-Athlete Medical Eligibility: Introduction

This webpage was created to assist you, the student-athlete, in the annual process of satisfying medical eligibility requirements as set forth by Southwestern College Athletic Training. It is the responsibility of the student-athlete to complete all relative medical history information, be aware of the general information in regards to Southwestern College Athletic Training Polices, and have completed the forms produced by Southwestern College.  

Introduction to the PPE

The annual pre-participation medical evaluation process includes the following components. Each component must be completed prior to satisfy medical eligibility. Incomplete medical evaluations will result in medical ineligibility and the student-athlete will not be allowed to practice and/or compete until this evaluation has been determined to be complete by the Athletic Training Staff. Each component in the pre-participation evaluation has been delineated below.

  • Step 1 - Read student-athlete general medical information
  • Step 2 - Complete the e-PPE online
  • Step 3 – Fill out Southwestern College forms  --  save, read, type in the blanks, and save to your computer AGAIN as a PDF file.
    • As required by e-signature, the document must be downloaded to your computer and then uploaded to the e-PPE website
    • If you are under 18, you will have to PRINT OUT ALL FORMS so that your parent or guardian can sign them, regardless of the e-signature.
  • Step 4 - Complete a physical examination with an MD, DO, PA-C, or ARNP
      • Southwestern College's Physical Form
      • e-PPE Medical History Summary
