Southwestern College Student Athlete:

On this website you will find all documents needed to start the 2011-2012 school year as a Moundbuilder Student-Athlete. I encourage you to fill these forms out with the help of your parent(s) or guardian. ALL documents MUST be typed out in order to be accepted. You are encouraged to get your physical by your family doctor** before returning to campus, but if you do not there will be a mass physical day offered upon your arrival to campus.

Southwestern College carries a secondary insurance policy for all student-athletes. This secondary policy is only good for current student-athletes who are hurt during a collegiate practice or competition. It will not cover medical expenses unrelated to official athletic participation. With the overall health and well-being of our student-athletes in mind, we require our student-athletes to be properly covered. If you already have an existing primary insurance, please make copies of the front and back of your insurance card and submit it with the rest of your forms. Even if your insurance has not changed from last year, we still must have these documents on file.

Please be aware of some changes that have taken place from previous years in our paperwork.

1) **We will only accept a physical from the following: a Physician (MD or DO), a Physician Assistant (PA-C), or a Registered Nurse Practitioner (ARNP)

2) Paperwork must be typed out-open in Adobe, and click the highlight button to find which blanks need to be filled in.

3) All paperwork must be done before you participate in ANY athletic practice, conditioning or event at Southwestern College
